Postural orthostatic tachycardia syndrome (POTS) is a debilitating condition traditionally characterized by a significant increase in heart rate upon standing.  This condition affects a disproportionate number of women in their teens and early twenties, though men and older women can develop the condition.

Below are the results experienced by a 22 year-old female patient that went through an intensive, 2 week program with a one week follow up program 6 weeks later.

This 22 year-old female patient reported the onset of symptoms in December 2010, including dizziness, lightheadedness, blurry vision, headaches, soreness in feet and legs, pre-syncope, gastrointestinal pain, fatigue, tremors, temperature intolerance, and brain fog. Prior to that, she had Swine Flu in 2009 and a viral infection in 2010. She was diagnosed with vasomotor instability in April 2011, and then with postural orthostatic tachycardia syndrome (POTS) in March 2012. She came to the POTS Treatment Center in Dallas, Texas in May 2013 for treatment. At that time, she was unable to drive, was using a wheelchair, and was having convulsive syncope. She experienced significant car sickness as well. She reported gaining 25 pounds in the prior year, with 15 of that being in the prior 6 months.  She had difficulty with activities of daily living, including eating, getting ready to go somewhere, and going to the restroom. Her ability to socialize was also negatively impacted. She was a freshman in college, but had missed 15 days of classes due to symptoms. She described herself as a high achiever, being active previously in marching band and dance.

Her treatment for POTS consisted of an intensive two-week program with sessions 5 days a week averaging 3 hours per session. Training included diaphragmatic breathing, Heart Rate Variability Training, EMG biofeedback, peripheral hand temperature biofeedback, EDR biofeedback, standing and exercise techniques, nutrition supplements, and other health and behavior interventions. She reported feeling an initial improvement in symptoms after 4 days of treatment. After the first week of treatment, she felt she did not need her wheelchair as much. After the two-week treatment program, she reported a large decrease in gastrointestinal issues and brain fog, and denied any tremors or seizures.

Her Beck Depression Inventory – II score pre-treatment was 11, indicating mild to moderate symptoms of depression. Her Beck Anxiety Inventory (BAI) pre-treatment was 36, indicating severe symptoms of anxiety. After the two-week treatment program, her BDI-II score decreased to a normal range score of 3, and her BAI score decreased to 17, indicating mild symptoms of anxiety.

This patient returned for follow-up treatment 6 weeks later. At that time she did an intensive one-week of training, with daily sessions of about 3 hours. Her POTS treatment again included diaphragmatic breathing, Heart Rate Variability Training, EMG biofeedback, peripheral hand temperature biofeedback, EDR biofeedback, standing and exercise techniques, and other health and behavior interventions. Upon her return to treatment, she reported that she had only used her wheelchair 3 times, was exercising up to 45 minutes at a time, and had been able to ride a bicycle for 6 miles since her initial treatment. She also reported she was able to control her car sickness better. She did report difficulty standing for long periods of time, though she was able to walk around. At the time of her follow-up her BDI-II score was lower at 1, and her BAI score was still 17.

As of September 2016, 3 ½ years after her initial treatment at the POTS Treatment Center, she reported that she had not used her wheelchair since her 6 week follow up at the clinic. She was also training for a half marathon and running several miles a day. She reported that she had not passed out in 2 ½ years. She was nearing completion of her degree and was working long hours on internship, but had not missed any work except when she had strep throat and mononucleosis.

Chances are that your financial state isn’t all that appealing. There are probably many bills to pay, debts to settle, and little to no income to deal with all of that. That’s definitely a lot to handle, and if you’re not careful, it can lead to a relapse.

The good thing is that financial stress can be managed as well. The first step is getting a job so that you have an income to slowly pay off everything that needs to be paid off. Next step: do your absolute best to not generate any more debt. This requires budgeting. You want to sit down and map out how much money you have, how much you owe, how much you will need to spend on a weekly to monthly basis, and how much you will be or are making. If you have extra money that is not already allotted towards necessary things, put that in savings. You can set short term and long term savings goals to help motivate your newfound financial responsibility.

Celebrations

This is also a big one. Holidays, birthday parties, what have you. They can be especially nerve wracking to former addicts. On the one hand, you probably associate having a good time and celebrating with alcohol and other substances. On the other hand, temptation is all around you. People that are not aware of your recent life choices will probably innocently offer you a drink, and even those that do know may forget or just insist that ‘a little sip never hurt anybody’.

There are many ways to deal with this. If you are in early recovery, the best thing to do would be to avoid events where you know that alcohol will be served or made available. If you are in long term recovery, or if you have a legitimate reason not to miss a certain event where there will be temptation you can try one, or a few, of these tricks:

Go to an AA or NA meeting before the event. This can help reinforce everything you’ve learned, and everything you’ve worked for to get where you are in your recovery.

Take some recovery tools with you. You can download an inspirational talk or two, or even a recovery app, that you can discretely use when you are feeling tempted or vulnerable.

Take a sober plus one. This person should be very firm in their abstinence, someone who is in long term recovery and is very confident of their sobriety. This way, you can have an anchor, and a third voice to remind you and encourage you to say no.

Have a reason for not drink. If you’re not comfortable with divulging this kind of information in public, be ready with some sort of response as to why you’re not drinking. It doesn’t have to be elaborate and it doesn’t even have to be a lie. You can just say “I don’t drink.”

Leave early. The intoxication part usually comes during the second half of an event, or nearing the end. You can just take that as your cue to leave so you don’t subject yourself to temptation.

Old cues

Once you make the decision to beat addiction, the importance of distancing yourself from triggers such as hang out spots, places you used to buy drugs or alcohols, people you used to use with, or even certain media that would put you in the mood to use (i.e. music and movies), cannot be stressed enough. Just being contact with these triggers, or cues, can not only remind you of the substance itself, but the need to use.

When your resolve is shaky, or you are at the beginning of your recovery, it is best to simply avoid these cues. It may seem like you don’t have anything left but that is not the case. You can create new habits and new routines. Join a class, a club, pick up a new hobby, do anything else that makes you happy.

As you go further into recovery, and are more sure of yourself, you can revisit your cues and practice not giving into temptation. Think of Pavlov’s experiment. Every time he fed a dog, he would ring a bell a few moments before the food was put in front of it. Overtime, the dog started salivating at the mere sound of the bell rather than when he saw the food. That’s conditioning. For you these people, places and things may trigger memories and even urges of alcohol and drugs. That is because, for a long time, every time you met up with certain people or went to certain spots, you would use. If you are able to revisit these cues multiple times without using, you are reconditioning yourself to not be triggered.
